1095-C Form Information
Form 1095-C is required by the Affordable Care Act (ACA) and is a certificate of Employer-Provided Health Insurance Offer and Coverage. Beginning with tax year 2015, this form is required by all large employers to report offers of health coverage and enrollment in health coverage.
Employees who were eligible to receive health insurance will receive a Form 1095-C as certification of the month(s) in which they have received the minimum essential health care coverage under Northwestern's benefits plans.
Common questions about the 1095-C
What do I need to do with my 1095-C?
Northwestern files directly with the IRS; the the 1095-C that you receive is for your information only. You should retain a copy of the form, but should not attach it when submitting your tax return. When completing your return, you will be asked if you had full-year coverage. Your 1095-C will provide the information you need to answer this question. You do not need your 1095-C to file your taxes, but you should keep it with your taxes for reference.
What if I did not receive a 1095-C?
Your form 1095-C for 2025 was printed and mailed by the end of February 2026. You will most likely receive your Form W-2 before you receive your form 1095-C. If you do not receive your form 1095-C by March 1, 2026 please contact Human Resources at askHR@northwestern.edu to request a copy.
