Health Care Form 1095-C

Form 1095-C provides information about the health coverage offered by your employer.  Anyone employed by Northwestern University in calendar year 2024 who was eligible for employee health insurance will receive Form 1095-C, including faculty, staff, postdocs, and retirees enrolled in the Legacy Retiree PPO, regardless if you enrolled in coverage.  Form 1095-C were printed and mailed on February 14, 2025 and will reach homes within two weeks of this date. When you terminate employment with the University, you may elect to temporarily continue to participate in certain University-sponsored benefits through COBRA or to convert certain coverage to an individual policy directly with the sponsoring insurance company.
