- $3,000 annual maximum benefit per member
- $50 per person annual deductible, $150 family maximum
- Preventive services covered at 100%
- Primary services covered at 80%
- Routine fillings (amalgams and resins)
- Endodontics
- Periodontics
- Oral surgery
- Major services covered at 50%
- Crowns/inlays/onlays
- Prosthodontic service
- Implant services
Orthodontics
Orthodontics covered for children through age 25. Claims must be submitted to Delta Dental in order to obtain reimbursement. For clarification and/or predetermination of the rates, please contact Delta Dental directly at 800-323-1743.
Enhanced Dental Benefits for Special Health Issues
Enhanced Benefit Program offers additional coverage for individuals who have specific health conditions (including pregnancy, diabetes, high-risk cardiac conditions, suppressed immune systems, and specialneeds) that can be positively affected by additional oral health care. Find a dentist
Members may receive care from any licensed dentist, but benefits are higher if care is provided by a Delta Dental PPO network dentist. Additionally, a dentist in the Delta Dental Premier network are considered in-network under the plan, you receive the largest discount if you see a Delta Dental PPO network dentist
