This is a summary of the plan benefits. Complete benefit information is available in the Certificate of Coverage [will be available closer to the start of the plan year], Benefits Booklet[will be available closer to the start of the plan year], and Summary of Benefit Coverage (SBC) 2025 [Spanish].
Find a doctor
- Navigate to the UHC Find a Doctor website
- Select Search as a Guest found on the right side of the page
- Select either Medical Directory or Behavioral Health Directory based on the type of care you need
- Select Employer and Individual Plans
- Scroll and select Navigate HMO
- Select Change Location, enter your ZIP code in the search bar, click Update
Location.
To change your PCP and OB/GYN to a different medical group, contact HMO member services at 855-828-7715.
If you are out of your area, for example, on vacation, a business trip, or as a college student attending school in another state, and need non-emergent care, first contact your selected PCP. They can provide a referral to a local network provider in the area, ensuring you have coverage within your Navigate plan.

Prescription coverage
- CVS Caremark - administered prescription plan
- Use CVS Caremark online pricing tool to price current or future medications. Shopping around may save you money.
Immunizations
- $25 copay
Virtual Visits
UHC offers access to virtual care for a variety of care needs. To get started, sign in at myuhc.com/virtualvisits, call the number on your health plan ID card, or download the UnitedHealthcare app.
- 24/7 Virtual Visits are a way to schedule same-day, urgent care visits, so you can talk with a provider 24/7 for common urgent care needs or when your primary care provider (PCP) is not available.
- Access to primary care and specialist providers without the office visit.
Routine Vision Care
- Eye exam every 12 months, covered with $0 copay
- Hardware such as glasses and contacts are not covered
Routine mammogram and pap smear
- Mammogram covered in full when ordered by Primary Care Physician (PCP) and performed according to American Cancer Society guidelines.
- Pap Smear covered in full within network.
Laboratory tests and x-rays
- Covered 100% if performed in a physician's office; subject to copay.
Physical therapy and chiropractic care
- $35 copay.
Minor surgery in doctor's office
- $25 copay.
Outpatient surgical operations
- After $250 per occurrence, covered in full with PCP referral.
Diabetes treatment
- Covered, subject to location of care and copay deductible.
- Supplies covered under prescription drug program. Equipment covered as durable medical equipment.
- Benefits are provided for outpatient self-management training, education and medical nutritional therapy.
Exclusions
- Routine vision care hardware (glasses, contacts)
- Custodial care
- Dental care
- Long-term care
- Non-emergency care when traveling outside the U.S.
- Private-duty nursing

- Hearings aids coverage available
Premiums are deducted from your paycheck on a pre-tax basis. Employees who are paid bi-weekly will have half of the monthly deduction taken from the first two checks of each month.

For new hires, the salary tier is determined based on your initial, regular salary. The salary tier for current employees is determined by your salary on September 1st of the year proceeding the plan year (e.g. Premiums starting 1/1/2025 are based on your 9/1/2024 salary). Mid-year changes to salary will not change your assigned salary tier, unless you transition from full-time to part-time or part-time to full-time. When your 9/1 salary crosses a salary tier due to a salary increase not associated with a change in full-time/part-time status, you will pay the lower premium for the following year.
