- $3,000 annual maximum benefit per member
- $50 per person annual deductible, $150 family maximum
- Preventive services covered at 100%
- Primary services covered at 80%
- Routine fillings (amalgams and resins)
- Endodontics

- Major services covered at 50%
- Crowns/inlays/onlays
- Prosthodontic service

Enhanced Dental Benefits for Special Health Issues
This program provides enhanced benefits to members with heart disease, diabetes, or who are pregnant.
